Florida Youth Soccer Association (FYSA), requires each player to have a signed COVID-19 Waiver and Concussion Awareness form on file in order to participate this season.  

We've made it easy for you with the following e-signature process.  Just type in your info for each field and hit "Continue".  At the end of the process, you and the club will receive an email with a copy of your signed waiver.
